If you require a doctor and the surgery is closed and you cannot wait until we re-open: - please contact NHS 111 Service.  This is an Out of Hours GP service and calls to 111 are free from Landlines and Mobiles.

New Patient Registrationregistration

The doctors welcome new patients who live within our practice area of BD1, BD5 and BD7. You will be required to attend an interview with our healthcare assistant prior to completion of your registration where you will be asked questions about your past health and current lifestyle. You will also be offered a health check.

Medical treatment is available from the date of registration.  Please contact reception for further information.

 

If you are already registered with us and move outside postcode areas BD1, BD5 and BD7, you may be asked to register with a Doctor in your new area This will ensure you are provided with appropriate, accessible and continued health care services.

 


You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.  We also need you to complete our Patient Questionnaire about your health and general lifestyle.  this will be required to complete your registration request, please bring the completed form with you.
